Service Description

For those that are happy with their overall care but have a specific medical condition that needs a more thorough review and treatment plan. The Focused Care Consultation will touch base on some aspects of the patient's total medical history, but is intended to provide a deeper understanding into a single medical condition, guidance on specialized testing, and customized treatment plans. This exam includes an initial appointment and a 6-12 week follow-up (depending on condition and treatment plan) to monitor progress. ** Please schedule an Initial Case Review to see if the specific condition is something we can assist with. * This exam is automatically included at no additional cost for all Enrolled Patients * Labs, imaging, and treatment orders/prescriptions may be issued as part of this service, but the cost of obtaining those services will be the responsibility of the patient.
